Please DO NOT show up for testing at the nurse's office without following the steps notated below.
2. Include the name of the individual needing testing (this is for staff and students only), phone number, description of situation; if the individual is a student also include grade and parent information (name and phone number)
3. The testing administrator (i.e. Nurse) will call you back as soon as possible; DO NOT go to the nurses office unless asked to do so by the nurse.
4. The testing administrator will let you know if you qualify for testing.
5. The testing administrator will guide you on where to go next to register for COVID Rapid Testing (staff will use a link and parents will complete a permission slip).
